Academic Integrity Form
Any time an instructor believes you have violated academic integrity, the process begins with this document. Each college and campus has their own version of the form. The example linked to below is from the College of Health and Human Development.
Take a moment to review the form. You'll notice the document has two sections. The first is information about the process. The second section is the form itself, which must be signed by both the student and the instructor.
